Surgical Procedures

Note: All surgical procedures require a consultation for scheduling. Preoperative planning, a review of the procedure, and final cost are discussed at that time. Pathology costs are included in the procedure fee.

  • Surgical treatment for a benign or malignant skin lesion (skin cancer, lipoma, cyst, etc...). This includes the excision procedure, repair with stitches, and subsequent suture removal.

    • Simple: $500

    • Intermediate: $750

    • Complex: $900

  • The use of one of a modality (electrodessication and curettage, cryosurgery, or other) to treat a skin cancer.

  • Surgical revision and repair of stretched piercing site, keloid or hypertrophic scarring, or distorted proportion.

    • Simple: $500 per ear

    • Complex: $750 per ear

Virtual Appointments in Texas, Georgia & North Dakota

Our aim is to provide safe and comprehensive care. Teledermatology appointments are limited to our medical services that can be evaluated effectively via our digital platform. At this time, we are unable to provide virtual consultation for spot checks or skin cancer screenings.

New Patient Visit

Important Financial Processes & Optional Fees

Because Dapper Dermatology is a direct patient care practice, we do not contract or negotiate with any third party payers. You may submit your costs to your insurance if you desire, but we do not promise or guarantee any reimbursements for services rendered. Our prices are subject to change at anytime - please refer to our website for current fee schedule.

Cost Per Prior Authorization - $50

Patients may work with their insurance to obtain their own prior authorization free of charge.

Employment Paperwork - $50

FMLA, FAA or other documentation.

Coded & Itemized Medical Receipt - $50

A non-coded receipt is complimentary.

  • You have the right to a good faith estimate to explain your expected medical care costs, including non-emergency items and services, if you don’t have insurance or choose not to use it.

    This estimate should cover all related expenses, such as tests, drugs, equipment, and hospital fees, and must be provided in writing at least one business day before the scheduled service.

    If the final bill exceeds the estimate by $400 or more, patients can dispute it. It's important to keep a copy of the estimate for reference.

  • Patients must be at least 18 years or older, or accompanied by a legal guardian.

    Patients must consent to and authorize an examination by their doctor and such assistant or staff as may be assigned by the physician.

    Patients may authorize Dapper Dermatology to fax my records to any physician or pharmacy for the purpose of coordinating or managing their healthcare. In order to do this patients may need a valid insurance card and a driver’s license or other legal form of identification at the time of the visit. Patients are responsible for knowing their insurance coverage and benefits. Dapper Dermatology is out-of-network with all private insurance companies and the patient is responsible for the cost of their visit as detailed on the website for services rendered. Payment will be required at the time of the visit via the credit card on file with Dapper Dermatology.

    For cosmetic services not covered by health insurance, charges are payable on or before the day service.

    Cosmetic services and skincare purchased directly from Dapper Dermatology are non-refundable services.

    Photography is at times a necessary part of planning and evaluating treatment. The patient or responsible party authorize the taking of photographs at the direction of the physician and/or delegate, solely for documentation purposes and recognize they will be kept confidential unless otherwise disclosed.

    Dapper Dermatology shall be entitled to recover any losses or damages it may suffer by reason of a failure of the patient and/or responsible party to pay charges when they become due, including, but not limited to, reasonable attorney fees, credit card disputes, plus costs of enforcing this agreement. Any amounts overdue for more than thirty (30) days shall accrue interest at the rate of 1.5% per month. Balances delinquent more than 90 days are subject to collection efforts and associated reporting to collection agencies. Patient will be responsible to pay Dapper Dermatology for fee’s charged by assigned collection agency. Patients authorize the release of any information needed for processing of this or any related claim/s. Patients will permit a copy of the authorization to be used in place of the original. Patients understand that all outside laboratory testing will be billed from the specific laboratories to them and/or their insurance company. Patients accept payment responsibilities if their insurance denies payment. A copy of this authorization shall be considered as valid as the original.

    Patients acknowledge they have read this information thoroughly and understand this patient financial responsibility form.

    If it becomes necessary to cancel or change an appointment, Dapper Dermatology requires at least 24 hours advanced notice (48 hours for a surgical procedure). This is important so that we may offer appointment time to another patient in need of seeing the physician. If an appointment is cancelled or changed with less than 24 hours’ notice (48 hours for surgical procedures), the patient will be charged in full for their scheduled visit. These fees will also be applied to patients account for any appointment no-show or late arrival. These fees will be the responsibility of patient or party financially responsible for patient.

  • The purpose of this policy is to ensure that all patients have a convenient and efficient way to pay for their medical services.

    1. Requirements

    All patients are required to keep a credit card on file with Dapper Dermatology. The credit card must be in the name of the patient or the patient's authorized representative.

    2. Use of Credit Card

    The credit card on file will be used to pay for any unpaid balances including late cancellation, no-show, and late arrival fees.

    3. No-Show Fees/Late Cancellation Fee

    Patients who cancel their appointment with <24 hours notice (<48 hours notice for surgical procedures) or who no show their appointment will be responsible for the full cost of their selected service. For patients who arrive late to their visit, they will still be responsible for the full cost of their scheduled visit.

    4. Credit Card Disputes

    To avoid fraudulent payments disputes + cancellations a nominal fee of $200.00 will be assessed in all cases where Dapper Dermatology has followed all stated protocols and procedures.

    5. Changes to Credit Card Information

    Patients are responsible for keeping their credit card information up to date. If the credit card on file expires or is lost, the patient must notify Dapper Dermatology.

    6. Security

    Dapper Dermatology takes the protection of all patient information seriously. We utilize Jane App and Stripe to store and protect patient health and financial data.

Laboratory Orders - $20 per lab

We provide flat rate laboratory services with our partner lab CPL.
