What Are Hyperpigmentation and Melasma?

Melasma is a skin disorder that results in uneven dark marks on the face. Underlying factors that contribute to it include sun exposure (both UVA and UVB) and medication changes such as birth control. The hormones estrogen and progesterone appear to play a role in worsening melasma, which is why changes in birth control and pregnancy are both known triggers.

Hyperpigmentation references dark marks that are left behind after skin injury. We commonly develop dark marks on the skin following acne breakouts, eczema, or psoriasis flares, or common cuts and bruises due to injury. Hyperpigmentation can be extremely challenging to treat, especially when it appears on the legs.

Sunscreen and sun protection are the two most important factors for melasma patients. From there, we add in prescription lightening agents among other topical ingredients. For severe cases, we may discuss prescription oral medications in addition to office-based procedures like lasers and chemical peels. Our favorite ingredients are topical retinoids, hydroquinone, tranexamic acid, kojic acid, glycolic acid, and alpha arbutin.
